প্রতি নির্দেশনা : ট্রামাডল, প্যারাসিটামল অথবা এ ওষুধের অন্য যে কোন উপাদান বা অপিয়ড -এর প্রতি সংবেদনশীল রোগীকে এ ওষুধটি দেয়া উচিত নয়। যে সকল ক্ষেত্রে অপিয়ড প্রতিনির্দেশিত, সেসব ক্ষেত্রে এ ওষুধটিও প্রতিনির্দেশিত।

সতর্কতা : এটি গাড়ী চালানো অথবা মেশিন পরিচালনার মত জটিল কাজ করতে যে পরিমান মানসিক ও শারীরিক দক্ষতার প্রয়োজন হয় তার ব্যঘাত ঘটাতে পারে। এই ওষুধটি অ্যালকোহলযুক্ত কোমল পানীয়র সাথে সেবন করা উচিত নয়। অন্যান্য ট্রামাডল অথবা প্যারাসিটামল যুক্ত ওষুধের সাথে এবং OTC ওষুধের সাথে একত্রে এই ওষুধটি সেবন করা উচিত নয়। ট্রানকুলাইজার, হিপনোটিকস অথবা অন্যান্য অপিয়ড এনালজেসিক এর সাথে সেবন করার ক্ষেত্রে সতর্কতা অবলম্বন করা উচিত।

শিশু এবং বয়স্কদের ক্ষেত্রে ব্যবহার : শিশুদের ক্ষেত্রে এ ওষুধটির নিরাপত্তা ও কার্যকারিতা এখনো নিরীক্ষা করা হয়নি। সাধারণত বয়স্কদের ক্ষেত্রে মাত্রা নির্ধারণে সতর্ক হওয়া উচিত। সমসাময়িক অন্য রোগ এবং এ সকল রোগের বিবিধ চিকিৎসার কারণে যকৃত, কিডনি অথবা হৃদপিন্ডের কাযর্কারিতা হঠাৎ করে কমে যাওয়ার সম্ভাবনার উপর পর্যালোচনা করে মাত্রা নির্ধারণ করা উচিত।

বৃক্ক সংক্রান্ত রোগীদের ক্ষেত্রে ব্যবহার : প্যারাসিটামল বি.পি. ৩২৫ মি.গ্রা.এবং ট্রামাডল হাইড্রোক্লোরাইড বি.পি. ৩৭.৫ মি.গ্রা.।

বৃক্কের সমস্যাজনিত রোগীদের ক্ষেত্রে নিরীক্ষিত নয়। যে সকল রোগীদের ক্রিয়েটিনিন ক্লিয়ারেন্স ৩০ মি.লি./ মিনিট এর কম তাদের ক্ষেত্রে দুটি মাত্রার মধ্যবর্তী সময় বাড়াতে হবে এবং খেয়াল রাখতে হবে যেন প্রতি ১২ ঘন্টায় ২টি ট্যাবলেটের বেশি না হয়।

যকৃতের সমস্যায় আক্রান্ত রোগীদের ক্ষেত্রে: প্যারাসিটামল বি.পি. ৩২৫ মি.গ্রা.এবং ট্রামাডল হাইড্রোক্লোরাইড বি.পি. ৩৭.৫ মি.গ্রা.। যকৃতের সমস্যাগ্রস্থ রোগীদের উপর নিরীক্ষিত নয়। তাই এর ব্যবহার এ ধরনের রোগীদের ক্ষেত্রে নির্দেশিত নয়।

পার্শ্ব প্রতিক্রিয়া : এসথেনিয়া, অবসন্নতা, হটফ্লাস, ঝিঁমুনি, মাথাব্যথা, কাঁপুনি, পেটব্যাথা, কোষ্টকাঠিন্য, ডায়রিয়া, পেট ফাঁপা, মুখগহ্বরের শুষ্কতা, বমি, এনোরেক্সিয়া, উৎকন্ঠা, দ্বিধা, ইউফোরিয়া, র্নিঘূমতা, বিচলতা, সোমনোলেন্স, পু্ররাইটাস, র‌্যাশ, অতিরিক্ত ঘাম ইত্যাদি।

গর্ভবস্থায় ও স্তন্যদানকালে ব্যবহার : প্রেগনেন্সি ক্যাটাগরি-সি। গর্ভবতী মহিলাদের উপর এখনো কোন পর্যাপ্ত এবং সুনিয়ন্ত্রিত তথ্য পাওয়া যায়নি। কেবল ভ্রূনের ক্ষতির ঝুকির তুলনায় চিকিৎসায় মায়ের উপকারের পরিমান অতিমাত্রায় বিবেচিত হলে, কেবলমাত্র সে ক্ষেত্রেই গর্ভাবস্থায় এ ওষুধটি ব্যবহার করা যেতে পারে।

যেহেতু নবজাতক এবং ইনফেন্টদের উপর এ ওষুধটির নিরাপত্তা প্রতিষ্ঠিত নয়, তাই অবসটেট্রিক্যাল প্রি-অপারেটিভ মেডিকেশন এবং ডেলিভারী পরবর্তী এনালজেসিক হিসেবে নার্সিং মায়েদের ক্ষেত্রে এ ওষুধটি নির্দেশিত নয়।
